Your role in a nutshell:

Bridge-Builder: Collaborate with physicians and staff to ensure seamless care.
Data-Driven: Manage disease registries and track quality metrics to ensure patients hit their health goals.
Patient Champion: Help patients manage chronic conditions and achieve better health outcomes.
Clinical Leader: Lead pre-visit planning and champion evidence-based best practices.
