Primary Office Location: Park Rapids – Within the Northern Dental Access Center
THIS POSITION DOES NOT PROVIDE “WORK FROM HOME” OPTIONS.
Travel to other locations likely to include: Walker, Bemidji and Bagley
Reports to: Executive Director/Ruth Sherman and Supervisor Trista Hill
PURPOSE: Provide Information, Referral and Advocacy services concerning the area’s service options pertaining to Seniors, Youth, Adults, Single households and families.
Duties:
- Build professional working relationships with area providers; including Hubbard County DHS, Northern Dental Access Center, mental health providers, MNsure Insurance providers in the region, Hubbard County schools, area faith-based organizations, and other agencies.
- Demonstrate the ability to learn about, navigate and teach others to navigate the range of state and federal health care options including Medical Assistance, Minnesota Care, Medicare, and MNsure.
- Exhibit awareness and respect for the continuum of financial, cultural, racial and personal lifestyle choices that exist among clients, co-workers, and the general public.
- Actively pursue information about area resources and how to access those resources.
- Assist clients in assessing and prioritizing their needs and connecting them with appropriate resources.
- Display the ability to recognize and de-escalate volatile situations.
- Demonstrate compliance with data privacy policies, grant policy requirements and policies required by the employer and landlord.
- Provide accurate, timely and consistent documentation and reporting.
- Maintain regular and consistent office hours, welcoming walk-in clients and ensuring a comfortable environment for all people.
- Engage in community events where tabling and/or presentations allow for marketing of CRC services.
Pay: $21.00/hour $21.50 upon completion of a 6-month probationary period
Hours: 40 hours per week
32 hours (Monday thru Thursday) in Park Rapids, 8 hours (Friday) in Walker
Requirements:
- Must provide a valid driver's license and proof of car insurance.
- Must pass a background check provided through MNsure
- Proficient with Office 365, TEAMS, Zoom, Microsoft Word and Excel
- Capable of operating a laptop computer, telephone, scanner, and copy machine
- Master MNsure Navigator training and acquire a certificate demonstrating state certification
Education:
Post high school education is preferred; but human service experience is essential.
